    In this presentation, Dr. Ashvin Agarwal reviews posterior polar cataracts and complications that can occur during surgery, such as posterior capsule rupture and vitreous prolapse. His discussion includes the following steps to address such complications:

    • Make a small capsulorrhexis.
    • Create viscoelastic pockets all around.
    • Use only slow hydrodelineation (no hydrodissection).
    • Never rotate the nucleus.
    • Embed and prolapse the nucleus into the suprapupillary axis.
    • Perform a direct chop and emulsify 1 quadrant with lower settings.
    • Loosen cortex all around, then tease and release the polar component from its attachment.
    • Inflate the bag with viscoelastic and inspect for posterior capsule compromise.
    • Place the intraocular lens in the bag.
