    Dr. Hassan Khaled encounters a fracture of the phaco-probe needle during the emulsification of a hard dense cataract, characterized by a sudden loss of aspiration, altered ultrasound energy transmission, changes in the effectiveness of emulsification, and abnormal vibrations. In this case, the fracture was thought to be caused by mishandling of the phaco tip, including inadequate tightening that can then lead to instability and increased stress of the needle. Another cause of needle fracture may be the use of excessive force when dealing with especially hard cataracts.
