The following Category III codes were introduced January 1, 2022: 0687T, 0688T, 0704T, 0705T and 0706T. As Category III codes they are not valued by the Relative Update Committee (RUC). Coverage and payment are at the payers’ discretion. Verify coverage and possible need for prior authorization with each insurance plan.
