Required Information for Testing Services
What information is required when a patient is referred to our office from an outside physician for a testing service only?Answer:
For any tests delegated to ancillary staff, you need an order from the ordering physician. It should indicate what, where and why of the test:
- Test(s) to be performed
- Which eye(s)
- Medical necessity
When submitting a claim, consider who the payer is and append the -TC modifier to the test(s) performed for those payers that recognize this modifier as indicating that the technical component was performed.
