A 77-year-old-man, after a misdiagnose of angle-closure glaucoma which was treated with two laser iridotomies, sought a second opinion on ocular redness with eyelid swelling and an intraocular pressure of 35mmHg on maximal tolerated topic therapy. Note on the left anterior segment photo the distinct dilated corkscrew episcleral blood vessels due to retrograde venous drainage resistance. After fistula embolization there was a significant improvement in engorged vessels appearance (right photo) and an intraocular pressure control occurred.
