    A new patient was diagnosed with conjunctivitis secondary to a chemical irritant in the right eye. We billed as a new patient E/M with H20.11 acute toxic conjunctivitis of right eye and the claim was denied, why? 

    Answer:
    According to guidance under H10.21 in the ICD-10-CM tabular index you must “code first: (T51-T65) to identify chemical and intent.” Link the ICD-10 for toxic effects of substances chiefly nonmedicinal as the source first then the acute toxic conjunctivitis.
