  • Do We Use Modifier -JW with Residual Eylea?


    Our ophthalmologist reports that if we have Eylea remaining in the syringe after an injection, we should bill the waste with modifier -JW. Is this correct?

    Answer:
    The syringes are prepared with overfill to allow the delivery of the appropriate dosage. Overfill is not considered reportable waste with modifier JW per CMS. When a prefilled syringe of Eylea is injected, 2 mg/0.05 ml, the procedure note should state “residual medication less than 1 unit was discarded.”
    The claim should be coded J0178 – JZ, 2 units.
