    What is the recommended documentation for a visually significant cataract that requires surgery? Is a manifest refraction required for every cataract surgery or will a glare test suffice?
    Answer:
    Most payers require documentation of best corrected visual acuity by manifest refraction. Review the Cataract Documentation Checklist and also visit the Academy’s Cataract Surgery Documentation website for other documentation requirements.
