    Answer:
    Eylea HD, 8 mg/0.07 mL, should be reported with a not otherwise classified (NOC) HCPCS code until a permanent code is assigned.  The indications for this new drug include neovascular age-related macular degeneration, diabetic macular edema and diabetic retinopathy.  The frequency of injections varies from that of Eylea 2 mg and other anti-VEGF drugs and should be confirmed on the FDA label per indication.
