  • How to Find the Correct NDC


    I have no experience adding a National Drug Code (NDC) to a claim but need to for therapeutic Botox. How do I find the correct code?

    Answer:
    NDC numbers are listed on drug packaging in 10-digit format, however, to be recognized by payers, they must be converted to an 11-digit 5-4-2 sequence. On the CMS-1500 claim form in item 24a or EDI loop 2410: enter 11-digit NDC code in 5-4-2 format, proceeded by “N4” qualifier followed by unit of measurement (UOM).
