    We have heard about the modifier JZ and assume it is only for retina practices. Are there any other examples in ophthalmic practice?

    Answer:

    Yes, for any single-dose containers, vials and packages based on Food and Drug Administration (FDA)-approved labeling with discarded drugs less than 1 unit, append modifier JZ to the HCPCS code reported.

    Examples could include single-dose vials or packages of fluorouracil, Omidria and Mitosol, inserts or implants like Dextenza and Durysta. Drugs labeled as multidose vials are excluded from reporting modifier JZ or JW.
