Billing Routine Exams to Vision Plans
When coding exams to submit to a vision plan, do we omit CPT code 92015 refraction since we are billing and paid under one exam code?

Whether the refraction (92015) is billed separately or bundled with another code (i.e., 92004, 92014, S0620, S0621) is payer dependent. Each payer will have its own published policy.
