    Question: Is the office preparation of serum tears billable to insurance?

    Answer: Most payers do not have an allowable for autologous serum tears, and, therefore, the patient may be responsible for payment. It's best to preauthorize. Report CPT code 92499 Unlisted procedure or service with a description in box 19 on the Centers for Medicare & Medicaid Services 1500 form. For Medicare Part B patients, have the patient sign an ABN acknowledging possible payment responsibility, and then submit 92499 - GA. List autologous serum tears in box 19 of the CMS 1500 form.

    Consider working with the drug/pharmacy plan for the patient as well for compounded medications.
