Proper Coding for Map Biopsies
What is the proper coding for map biopsies of the left eye and left upper and lower eyelids? Eight incisional biopsies were performed in total: four bulbar conjunctival and four palpebra conjunctival.
CPT 68100 Biopsy of conjunctiva is the correct code and payment is per eye, not per incision. For Medicare Part B patients, when surgery is performed bilaterally, submit one line item with the surgical code appended with modifier -50, 1 unit and fees doubled. Other payers may require -RT and -LT to be appended.
