Answer: S0077 is a valid 2021 Healthcare Common Procedure Coding System (HCPCS) code for Injection, clindamycin phosphate, 300 mg or just “Injection, clindamycin phosp.” However, this HCPCS code is not on the current Centers for Medicare & Medicaid Services (CMS) Average Sales Price drug pricing files.
If the clindamycin was compounded, report with J7999 or J3490 and include the name of the medication, dosage and invoice amount in item 19 of the CMS 1500.
