When Should We Unbundle?
Question: When is it appropriate to unbundle CPT codes 67113 and 66982 or 66984?
Answer: It would be appropriate to use modifier -59 to unbundle these procedures when:
- It is documented preoperatively that both procedures need to be performed.
- The medical necessity for cataract surgery is documented preoperatively per payer policy.
- Separate incisions are used for each procedure.
The payer may require supporting documentation (preoperative exams and operative reports).
