High IOP or high bottle height phacoemulsification gives the advantage of volume and space for manipulations. However, it has its own set of disadvantages. Low IOP offers the advantages of working near physiological pressure with less predisposition to posterior movement of nuclear fragments. Low IOP phacoemulsification has its own set of disadvantages. Through intelligent use of different IOPs in phacoemulsification, one can make the most of all these advantages.
