This video on posterior polar cataract will show preoperative diagnosis; anterior segment optical coherence tomography (OCT) for status of posterior capsule (PC); PC protection with stable chamber, low IOP, epinuclear shield, irrigation/aspiration and IOL implantation; layer-by-layer separation in manual and femto delineation in femtosecond laser–assisted cataract surgery in soft cataracts; no hydroprocedure, no rotation, cross chopping, lateral sculpting, mechanical and visco separation in hard cataract; continuous curvilinear capsulorrhexis (CCC) size, nucleus removal, posterior CCC and optic capture in posterior polar cataract with posterior capsular rupture; and intraoperative OCT-guided phacoemulsification showing intraoperative dynamics and safety.
