The PreserFlo MicroShunt is a new device designed for treating patients with open-angle glaucoma. It is composed of a biostable thermoplastic elastomer called SIBS (styrene-block-isobutylene-block-styrene), which involves minimal inflammation or encapsulation compared to other implant materials. The suggested way of implanting the device involves creating a small scleral pocket with a blade, sliding a 25-gauge needle into the pocket to perforate the anterior chamber and flushing the stent with a 23-gauge thin-wall cannula. However, this method can create false channels and allow for significant filtration of aqueous alongside the MicroShunt, with the risk of hypotony. This video proposes a simplified implantation method that involves using the 25-gauge needle to make the scleral tunnel and enter the anterior chamber, followed by assembling the MicroShunt on a 23-gauge cannula and guiding it into the scleral tunnel while flushing water. This method has several advantages, including faster procedure, better control and less risk of bleeding.
