Use of 14-, 15- or 16-Gauge Indwelling Needles for the Removal of Large Soemmering Rings

Cataract/Anterior Segment, Complications, IOLs and Implantation

Removal of a large Soemmering ring with passive flow via a small cornea tunnel incision is considered difficult. This video demonstrates a novel technique of removing Soemmering rings using 14-, 15- or 16-gauge (G) indwelling needles in 10 eyes and illustrates the differences noted in the use of these needles. During the surgery, the surgeon connects the indwelling needle to the active extrusion line of a vitrectomy machine and inserts it via a cornea tunnel incision (16G, 15G:2.5 mm; 14G:3.5 mm). The surgeon is able to remove the large solid Soemmering rings by applying suction pressure to the needle and crushing it with an ophthalmic hook. No intraoperative complications are observed. This technique will allow surgeons to remove various Soemmering rings safely and efficiently via a small cornea tunnel incision. Additionally, indwelling needles are cost-effective and easy-to-use tools for ophthalmic surgery.
