Primary angle-closure disease is a common cause of glaucoma, particularly in Asian populations.

Although pupillary block is the most common mechanism, other causes, including plateau iris and lens-associated factors, often play a role.

Gonioscopy should be performed in all patients whom the clinician suspects of having glaucoma or narrow angles, including those with hyperopia and older phakic patients.

Treatment is tailored to the individual patient and can include medical therapy, laser iridotomy, laser iridoplasty, and lensectomy.
