    To treat a pediatric patient with unilateral buphthalmos and high myopia who continued to have distorted vision a year after undergoing trabeculotomy and trabeculectomy, Dr. Jagat Ram and colleagues created a customized "mega-implantable" phakic lens and inserted it between the iris and the crystalline lens. This phakic lens needed to be larger than normal because the patient's sulcus-to-sulcus length was 14.55 mm, greater than the standard phakic IOL length of 12.75 mm.
