Dr. Amar Agarwal presents an IOL scaffold technique that prevents nucleus fragments from dropping into the vitreous after posterior capsule rupture. An IOL is inserted and placed under the nuclear fragments and over the iris, with the leading haptic resting on the iris and the trailing haptic in the incision site. Removal of nucleus fragments and phacoemulsification may then proceed. Next, the IOL is repositioned into the ciliary sulcus and surgery concludes normally. The technique may also be used for IOL exchange and safe removal of Soemmering’s rings.
