    Angle kappa can make the difference between a happy and an unhappy patient in a multifocal IOL. The key is not to have a well centered IOL on the pupil but on the visual axis. In complicated scenarios like posterior capsular ruptures a glued multifocal IOl can also be implanted and the IOL adjusted on being centered on the visual axis.
