    Purpose: Management of iris defects is always challenging. Methods: We demonstrate possibilities of modern iris reconstruction focusing on a special customised iris to correct for partial or complete aniridia.Results: In a case of an ectopic pupil a central opening was cut with a 23 gauge vitrector. The artificial iris was implanted creating a new pupil with excellent postoperative results. Conclusion: Different implants are available for total or partial iris reconstruction and the aesthetic result improved significantly due to customized colour selection providing high patient satisfaction postoperatively.
