    Dr. Ahmed Assaf uses femtosecond laser to create a capsulotomy for an uncomplicated cataract surgery case. Following uneventful docking, he notes a tag in the upper temporal quadrant of the capsulotomy that extends into a full anterior capsular tear. Using gentle circumferential maneuvers, he manages to remove the capsule and proceeds with hydrodissection and nucleus removal. Since irrigation and aspiration at the site of the tear could cause further damage, Dr. Assaf switches to dry aspiration for cortical removal. Despite great care, however, the tear expands into the posterior capsule. Fortunately, there was no vitreous prolapse, and the surgeon finds sufficient capsule to support a single-piece acrylic IOL inside the bag.
