Cataract Surgery with Zonular Instability

Drs. Gabriel Valerio and Joseph Schmitz present a case of zonular instability that was fixed with a variation of the Canabrava technique, utilizing capsular tension hooks, a ring, and two segments. The patient had retinitis pigmentosa and an associated posterior subcapsular cataract, along with phacodonesis and a very shallow anterior chamber. Because of zonular weakness in the capsule, a cystotome was ineffective, and low-amplitude, high-velocity strokes were needed to pierce the anterior chamber.
