    Dr. Rahul Pandit performs an IOL exchange in a patient who complained of persistent photopsias following implantation of an Acrysof toric IOL. The surgeon uses viscodissection to loosen the lens haptics and optic from the capsular bag. Once the IOL is free, Dr. Pandit uses Vannas scissors to make two perpendicular radial cuts in the optic, and then removes the resulting quarter slice of the optic. This "Pacman" configuration allows the IOL to be easily rotated through the main corneal incision and removed. The capsular bag is expanded with a viscoelastic agent and a Tecnis 1-piece monofocal IOL is inserted.
