A patient presented with a calcified silicone optic in the setting of asteroid hyalosis, leading to decreased visual acuity. To fix this, Dr. Morgan Micheletti performed an IOL exchange with a pars plana approach to anterior vitrectomy. As silicone IOLs become more prevalent in light-adjustable IOLs, it is important to remember the association between silicone IOL calcification and asteroid hyalosis.
