    Dr. Rupert Menapace discusses his alternative to in-the-bag IOL implantation: buttonholing the IOL into the posterior capsule. His experience with more than 1,000 cases shows the technique prevents posterior capsular opacification and significantly reduces fibrosis. The technique applies to both adult and pediatric patients with pseudoexfoliative glaucoma, high myopia, or peripheral retinal disease, and may have a future with toric IOLs.
