Central vision deteriorates in each eye over weeks to months, but there is no pain; optic discs may appear normal or hyperemic and swollen. Physicians should consider an LHON diagnosis in any patient with painless, subacute optic neuropathy, especially if it is bilateral, and immediately perform blood testing.
