    Malyugin Ring Challenges

    Dr. Ayan Mohanta encounters a challenging scenario during cataract surgery with a Malyugin ring. Following phacoemulsification, the posterior capsule ruptures, necessitating a vitrectomy. During the removal of the vitrector, the Malyugin ring is partially externalized through the side incision. At this point, the patient begins to complain of pain, and rapid eye movement makes it extremely difficult to remove the ring. Dr. Mohanta tries several maneuvers before he successfully removes the Malyugin ring using the injector. He finishes the surgery with a hydrophobic IOL, and triamcinolone in injected into the anterior chamber to ensure there is no remaining vitreous.
