In this surgical video, Drs. Andre Witkin and Xuejing Chen manage a patient with a giant retinal fold. The fold had developed after the patient underwent a vitrectomy, laser and C3F8 gas for a giant retinal tear and retinal detachment by another surgeon. One month after that procedure, the retina was flat with a persistent giant fold through the macula. Drs. Witkin and Chen attempted to re-elevate the retina with forceps, and use a flex loop to gently detach the edge of the retinal tear. Once the retina was completely unfolded, perfluoron was used to reflatten the retina. Complete air-fluid exchange was performed, followed by an injection of silicone oil.
