A 56-year old woman presented for evaluation and management of a recurrent rhegmatogenous retinal detachment with proliferative vitreoretinopathy in the left eye. She underwent scleral buckle placement, pars plana vitrectomy, pars plana lensectomy with sparing of the anterior lens capsule, membrane peeling, retinectomy, removal of subretinal bands, and silicone oil exchange. This case demonstrates the utility of combining pars plana vitrectomy with adjunctive scleral buckle to address predominantly posterior pathology.
