    Extraction of a Large IOFB by an Anterior Approach

    This video demonstrates a situation encountered during removal of a long intraocular foreign body (IOFB). A 41-year-old man presented with sudden vision loss in the left eye because of ocular trauma. The ophthalmological examination revealed a complicated corneal wound with post-traumatic cataract and an IOFB. Initially, patient benefited from a corneal suturing. In a second step after 72 hours post-trauma, simultaneous crystalline lens extraction, without implantation, and vitrectomy were performed. The vitrectomy was performed as completely as possible, with the release of adhesions around the IOFB. Subsequently, the IOFB was mobilized using vitreotome aspiration only. The lower part of the IOFB was placed on the anterior capsule, while its upper part was deposited on the iris. The corneal incision was enlarged and the foreign body was extracted through it. An endolaser retinopexy with silicone tamponade was performed after implantation of a foldable implant in the crystalline sac.
