Surgical Feeder Vessel Ligation and En Bloc Resection of Retinal Capillary Hemangioblastoma

    AAO 2023 Video Program
    Retina/Vitreous, Vitreoretinal Surgery

This video describes a novel technique: feeder vessel ligation and en bloc resection of retinal capillary hemangioblastoma (RCH). A 42-year-old woman was diagnosed with a solitary large RCH in the temporal quadrant with associated localized tractional retinal detachment and macular exudation. The patient's workup was negative for Von Hippel-Lindau disease. This video demonstrates a unique surgical technique of 25-gauge chandelier-assisted bimanual pars plana vitrectomy, highlighting feeder vessel ligation, followed by en bloc transscleral endoresection of the tumor. Histopathology showed mononuclear round cells with proliferating blood vessels, confirming the diagnosis of RCH. Surgical pearls during the various steps of surgery are highlighted in this video.
