Treatment of Macular Fold Following Retinal Detachment Surgery Using 41-Gauge Needle and Flex Loop

We describe a technique of resolving macular folds by inducing macular detachment and using a Finesse loop to unfold the macula. First, thick epiretinal membrane and internal limiting membrane were removed. BSS was then injected into the subretinal space to detach the macula using a 41-gauge needle. The folded macula was gently spread using the Finesse loop. Perfluorocarbon liquid was used to prevent refolding of the macula, a retinotomy was made to drain the excess subretinal fluid, and gas tamponade was performed. After surgery, the patient's visual acuity and subjective discomfort were improved significantly.
