In this prize-winning video, Fernando C. Trindade, MD, reminds surgeons of the importance of keeping both haptics of an AcrySof single-piece IOL (Alcon, Inc., Hünenberg, Switzerland) in the capsular bag since having one or both haptics in the sulcus can cause problems. He demonstrates an elegant solution for when one haptic ends up in the sulcus and the lens is off-center: He just cuts the haptic off.
It appears that both haptics are not necessary for IOL stabilization when the single-piece AcrySof IOL has been in the eye beyond the initial postoperative period and once the optic and opposite haptic are securely fibrosed within the capsule.
The MST Duet instruments (MicroSurgical Technologies, Redmond, WA) are featured in this video undertaking the unusual task of severing the haptic. These instruments are versatile and get the job done. Dr. Trinidade uses them to make a difficult surgical procedure look easy. When you have an intraocular procedure that would benefit from a closed system, think of using the Duet system or something similar.
