Bacterial keratitis history. Is the patient a contact lens wearer and has the following occurred: trauma, keratoconjuctivitis sicca, and blepharitis. Has there been structural alteration or malposition of the lids/lacrimal system and a decreased immunologic defense or a history of previous corneal disease/steroid use.
