This case report found that AS-OCT provides novel and detailed information of coin-shaped lesions in patients with cytomegalovirus (CMV) corneal endotheliitis and may be a useful adjunct for diagnosis and follow-up in these patients.

The authors describe for the first time the appearance of CMV corneal endotheliitison on AS-OCT.

They used AS-OCT to examine the in vivo corneal changes of coin-shaped lesions in two patients (69- and 71-year-old men) with CMV corneal endotheliitis.

In both cases, the coin-shaped lesions were observed at the corneal endothelial surface as clusters of fine precipitates on slit-lamp biomicroscopy. On AS-OCT, the lesions appeared as an irregularly thickened highly reflective endothelial cell layer.

Lesions resolved in both patients after anti-CMV treatment with oral valganciclovir, topical ganciclovir and topical corticosteroid drops.
