Capsulorhexis can be particularly difficult in conjunction with PKP whether performed prior to or after the host's opacified cornea has been removed. This video shows a technique of using Zepto open sky with a small pupil to achieve a round intact capsulotomy without the need for can-opener capsulorhexis. The posterior capsule is preserved for insertion of a secondary PCIOL either in the bag or in the sulcus once the keratometry is stable.
