Dr. Ken Wright shares a case of a posterior lenticonus. Following uneventful capsulectomy and cortical cleanup, the surgeon injects an IOL into the bag, which sinks to the anterior vitreous. In cases of posterior lenticonus, the posterior capsule may be diaphanous or entirely absent. The surgeon retrieves the lens and performs an anterior vitrectomy. A 3-piece IOL is then carefully rotated into the sulcus and each haptic is positioned behind the iris.
